Job summary

Baptist Medical Center South in Alabama is seeking a Discharge Planner (MSW) to coordinate care for an assigned caseload through collaboration with physicians, nursing staff and other health professionals. The Discharge Planner will facilitate efficient utilization of clinical resources, optimal clinical and continuing care, as well as financial outcomes.

• Master's Degree in Social Work required.
• Minimum 3 years of hospital experience preferred.
• Knowledge of case management.
• Experience with skilled nursing facility, home health, hospice, abuse/neglect, and rehabilitation preferred.
