Job summary

Mercy Medical Center in Springfield, MA is seeking a Licensed Discharge Planner / Social Worker (LSW/LCSW) to join our Case Management Team. The Discharge Planner collaborates with patients, families and the medical team to ensure safe discharge planning and optimal patient well‑being.

Responsibilities include psychosocial assessments, ongoing case management, connecting patients to community resources, and supporting transitions off the acute care setting.

Qualifications

• Bachelor's degree in social work required; Master's degree preferred.
• Massachusetts social work license required (LSW minimum); LCSW preferred.
• Two years of related professional experience; fieldwork may substitute for one year.

Responsibilities

• Perform psychosocial assessments to determine patient care needs.
• Provide ongoing case management support to patients.
• Help connect patients to resources within the community.
• Assist and support patients who require discharge planning services.
• Utilize extensive knowledge of community resources, managed care, and medical terminology.
