Mercy Medical Center in Springfield, MA, is seeking a Licensed Discharge Planner / Social Worker (LSW/LCSW) to join our Case Management Team. The Discharge Planner collaborates with patients, families and the medical team to ensure optimal recovery and well-being.

You will perform psychosocial assessments, provide ongoing case management, and help connect patients to community resources. A Massachusetts social work license is required, with an LCSW preferred.

Qualifications

• Bachelor's degree in social work required.
• Master's degree preferred.
• Two years of related professional experience required.
• Fieldwork placements may be substituted for one year of experience.

Responsibilities

• Perform psychosocial assessments to determine patient care needs.
• Provide ongoing case management support to patients.
• Connect patients with community resources.
• Assist with discharge planning and related processes.
