Job summary

Johnson Memorial Hospital is seeking a part-time LMSW Social Worker / Discharge Planner to join our Case Management team. The role focuses on assessing and providing direct Social Work services to patients and families during admission and discharge.

The position requires an MSW from an accredited program with LMSW preferred in CT, and CCM/ACCC certifications are a plus. Knowledge of Medicare/Medicaid and utilization management is beneficial.

Qualifications

• MSW degree from an accredited School of Social Work required.
• CT State Social Work Licensure LMSW preferred.
• CCM (Case Management Certification) preferred.
• ACC C (Continuity of Care – advanced) certification preferred.
• Knowledge of Medicare, Medicaid, and managed care regulations and trends.
• Experience with utilization management, home care, community resources, and rehabilitation beneficial.

Responsibilities

• Provide ongoing case management support to the patients.

Johnson Memorial Hospital is looking for LMSW Social Worker / Discharge Planner to join our Case Management team. The Social Worker is responsible for assessing and providing direct Social Work services and interventions to patients and families in the hospital setting upon admission and throughout discharge.

You Will Be Responsible For

• Communicate effectively with staff, colleagues, patients, and family members.
• Utilizes standard Social Work intervention techniques collaborating with patients and families.
• Work as a resource to other staff members.

Minimum Requirements

• MSW degree from an accredited School of Social Work required.
• CT State Social Work Licensure LMSW preferred
• Certification in case management (CCM) and certification in Continuity of Care – advanced (ACCC) preferred.
• Knowledge of Medicare, Medicaid, and managed care regulations and trends
• Experience with utilization management, home care, community resources, and rehabilitation beneficial.
