Qualifications

• Master’s degree in a health or social services field and one year of healthcare experience or 2 years in related roles.
• Strong leadership, communication and liaison abilities across disciplines.
• Experience with discharge planning and care coordination preferred.

Responsibilities

• Supervise LTACH ALOC and SNF referrals coordination with social workers and clinical staff.
• Coordinate community and government agency collaborations to support patient care.
• Develop strategies to improve discharge planning and service delivery.
• Oversee department administration and record-keeping activities.
• Lead interdisciplinary teams to optimize patient flow and care transitions.
• Plan and conduct data analysis on program metrics and outcomes.
