Job summary

East Carolina Health & Rehabilitation Center in Greenville, NC seeks a Discharge Planning & Social Work Assistant to ensure smooth transitions for patients returning to the community. You will work with the Director of Discharge Planning & Social Work to address emotional, social, and practical needs and connect patients and families with community resources.

You will advocate for residents’ rights, develop individualized discharge plans with interdisciplinary teams, coordinate post-discharge

Qualifications

Experience in post-acute social work, discharge planning, and patient relations.
Proficient with computer programs including Microsoft Office Suite (Word, Excel, etc.).
Excellent communication and interpersonal skills.

Responsibilities

Advocate for residents’ rights and preferences, ensuring discharge plans align with goals.
Collaborate with interdisciplinary teams to develop individualized discharge plans.
Coordinate post-discharge services, including home health care and community resources.
Assist patients in accessing and utilizing community resources, such as transportation and support groups.
Provide emotional support and companionship to patients.
Support the overall operation of the healthcare center.
