Job summary

Qualifications

• RN degree required; active MI RN license
• Two years clinical RN experience required
• Preferred 5 years Case Management experience

Responsibilities

• Assess, screen, and plan discharge from admission to post-discharge
• Document assessments and plans in EMR accurately
• Collaborate with UR team, physicians, and other providers to ensure appropriate care level and resources
• Advocate for patients and families, coordinating with outside agencies and payers
