Qualifications

• High school diploma required.

• Six months experience in a health care environment required.
• Preferred experience in discharge planning (HMO, hospital, home health or clinical setting).

Responsibilities

• Assist Social Workers and Case Managers with coordination of care for psychosocial/socioeconomic needs across the care continuum.
• Ensure timely care and regulatory compliance; promote efficient use of hospital resources.
• Complete established competencies for the position within the introductory period.
