Job summary

Universal Health Services, Cedar Creek Hospital of Michigan, is seeking a PRN Discharge Planner to support the psychiatric case-management department. You will initiate discharge planning, arrange aftercare, and handle discharge paperwork to ensure smooth transitions for adolescents and adults.

The role requires a Bachelor’s in Social Work, strong communication, and administrative experience; a flexible PRN schedule at our Michigan facility may be involved.

Position Description

The Discharge Planner/Coordinator (BSW) provides discharge planning support to the psychiatric case-management department and supports psycho-educational group programming. Key responsibilities include, but are not limited to:

• Initiates discharge planning and arranges aftercare appointments/referrals.
• Completes all needed discharge paperwork with patients and supports at time of discharge.
• May conduct psycho-educational groups and other groups as assigned.
