Responsibilities

• Provide grief counseling and crisis intervention for patients and families.
• Assess discharge needs across inpatient, observation, and ED settings.
• Review medical records to inform discharge planning and care transitions.
• Develop discharge plans with post-acute care arrangements.
• Coordinate communication with post-acute services and facilities.
• Participate in multidisciplinary rounds to align resources for discharge.
• Educate patients and families on illness impact and resource access.
• Organize and lead patient/family care conferences.
• Document evaluation, plans, barriers, and care progression.
• Advocate for patient rights and options during hospitalization.
• Coordinate payor authorization needs for post-acute care.
• Assess readmissions and identify preventable factors.
• Perform other duties as assigned.
