Job summary

Oceans Behavioral Hospital Greater New Orleans seeks a Discharge Planner to coordinate patient treatments with physicians, therapists and nursing staff. You will manage discharge assessments, schedule follow-ups and ensure timely transitions from inpatient programs to community resources.

Responsibilities include documenting care plans, communicating with families, completing required forms, and adhering to Joint Commission and state regulations.

Qualifications

• Associate degree in healthcare or related field; Bachelor’s preferred.
• Experience in psychiatric/discharge planning or hospital setting preferred.
• Knowledge of Joint Commission, state regulations and reimbursement processes.

Responsibilities

• Act as patient liaison to coordinate treatments with physicians, therapists and nursing staff.
• Review psychosocial data and prepare discharge assessments for multidisciplinary team.
• Schedule follow-up appointments and coordinate continuity of care.
• Communicate discharge planning activities to team; update attendance calendars.
• Complete referral paperwork and post-discharge care items; assist families with arrangements.
• Maintain accurate discharge reviews and data entry in the hospital systems.
