Summary:
The Discharge Planner role has responsibility to carry out a full range of aftercare and discharge planning services. The specific discharge planning needs of each patient shall be identified in collaboration with the unit treatment team and may include patient interviews, record reviews, and participation in network and discharge planning meetings.

The pay range for this position is $61,256.00to $82,867.20per year.
Compensation for part-time positions isdeterminedon a prorated basis consistent with the scheduled hours and applicable full-time equivalent (FTE).
