Job summary

Burrell Behavioral Health in Springfield, MO is seeking a part-time Discharge Planner to coordinate discharge planning from admission through post‑discharge follow‑up. You will develop individualized safety plans, schedule follow‑ups, and link clients to outpatient, community, and residential services.

You will document in the EHR, participate in case reviews, and help remove barriers such as transportation or insurance to promote continuity of care.

Qualifications

  • Bachelor’s degree in Social Work, Psychology, or related field required.
  • Master’s degree preferred for advancement or specialized roles.
  • Experience or coursework in crisis care, case management, or care coordination is valued.
