Job summary

Lakeside Behavioral Health System is seeking a Discharge Planner to provide discharge planning, case management, and crisis intervention within an inpatient psychiatric setting. This role involves liaison work with referrals and hospital departments to ensure smooth patient transitions.

Ideal candidates have a Bachelor's in Social Work or related field, with experience in psychiatric care. RN licensure is preferred where applicable. Competitive compensation and comprehensive benefits are offered.

JOB SUMMARY:

The Discharge Planner provides discharge planning and case management services, crisis intervention and liaison services with referral sources and other departments within the hospital in a manner consistent with the philosophy and objectives of Lakeside Behavioral Health System.

To perform this job successfully, an individual must be able to demonstrate competency in the criteria listed in this job description. The specifications listed below are representative of the knowledge, skill, and ability to meet the minimum requirements for this position. Staff must demonstrate flexibility for reassignment to units providing care to varied populations including, adults and dually diagnosed patients in an inpatient and partial hospital setting.

Education: Bachelor’s degree in Social Work, Clinical/Counseling, or Social Science related area required, licensure preferred. Graduate of an accredited nursing program.

Experience: Prior experience working with a psychiatric mental health population, preferably in an inpatient setting.

Licensure/Certification: If a Registered Nurse – must have licensure and registration by the Tennessee Board of Nurse Examiners as a registered nurse.

Knowledge: Prefer knowledge of case management, discharge planning, family therapy theory and practice.
