Job summary

Pinnacle Treatment Centers, Inc. is seeking a Discharge Planner to initiate and coordinate discharge planning for patients, scheduling aftercare and communicating with referral sources. You will work with the treatment team to ensure appropriate care levels at discharge and maintain contact with community resources.

The role requires 3–5 years of discharge planning experience and a Bachelor’s degree in a related field. Competitive benefits and career growth opportunities are provided.

• 3–5 years clinical case management or discharge planning experience.

Responsibilities

• Provide timely discharge planning from the facility, coordinating with Utilization Review to maximize reimbursement.
• Build and maintain relationships with referral sources and community agencies.
• Maintain an up-to-date list of referral sources and contact information.
• Deliver written discharge plans to patients and families, identifying suitable facilities or community resources.
• Assess patients' financial resources and coordinate transitions to providers.
• Document discharge plan in the EHR and complete final documentation within two working days.
• Work as a member of a multidisciplinary treatment management team.
• Ensure discharge plans include stable housing and coordinate with patients.
• Provide exceptional customer service to internal/external customers and support service standards.
• Attend team meetings and complete required trainings timely.
• Other duties as assigned.

As a Discharge Planner, you will be responsible for initiating discharge planning for patients, working closely with the treatment team to schedule appropriate after care appointments and communicating with referral sources. You will coordinate, conduct, and implement discharge planning for patients at the levels of care being provided upon discharge. This role also works as a part of the treatment team and maintains effective communication with teammates and community resources and accountability for services provided.

Responsibilities

• Provides appropriate and timely discharge planning from the facility, working closely with Utilization Review staff to maximize reimbursement and maintain appropriate census at each level of care.
• Responsible for creating and maintaining relationships with referral sources and community agencies providing services to persons with substance abuse and co-occurring mental health disorders, as well as, identifying the need to return clients to referral source at time of discharge.
• Maintaining a list of referral sources and contact information
• Provides written discharge planning to patients and families, determining which facility or community resources should be included in the patient's discharge plan, and requesting the determination of level of care for placement.
• Accurately assesses patient's financial resources, including insurance benefits, and coordinates transitions to providers or other referral sources as deemed appropriate.
• Documents and completes discharge plan in EHR, completing final documentation within two working days of discharge.
• Functions as a member of a multidisciplinary treatment management team and the clinical department.
• Works with patients to ensure a discharge plan, including stable housing, is set in place before discharge.
• Provide exceptional customer service to all internal/external customers with rapid response to inquiries and concerns & support facility internal and external customer service standards.
• Attend team meetings and complete all training courses timely as required.
• Other duties as assigned.
