Job summary

Northeast Center for Rehabilitation and Brain Injury is hiring a Discharge Planner to assess psychosocial needs, provide counseling, and develop comprehensive care plans to support residents’ successful community re-entry.

The role requires coordinating with interdisciplinary teams, residents, families, and guardians, and exploring discharge options and waivers to optimize post-discharge outcomes.

Qualifications

• BSW or related degree with one year of social work or case management experience.
• Professional attire and clinical communication skills.
• Experience with office technologies, computers, word processing and software programs.
• Strong organizational skills and professional telephone demeanor.
• Ability to read, write, speak and understand English.
• Ability to make independent decisions with patience and enthusiasm.

Responsibilities

• Assess psychosocial needs, adjustment to illness, and community re-entry barriers via intake/psychosocial assessments and care plans.
• Provide individual, group, and family counseling.
• Facilitate comprehensive care plans with interdisciplinary teams and designated representatives.
• Explore discharge planning and waivers (e.g., TBI, NHTD, OPWDD) and traditional services.
• Maintain communication with designated representatives and legal guardians.
• Interface with OASIS program, justice system connections, and other disciplines.
• Complete entitlements/benefits and participate in performance improvement.

We are seeking a highly motivated and experienced Discharge Planner to join our team at Northeast Center for Rehabilitation and Brain Injury. Our 39-acre campus in Mid-Hudson Valley offers a unique and supportive environment for individuals with cognitive, physical, and social barriers.

Job Summary:

As a Discharge Planner, you will be responsible for assessing psychosocial needs, providing counseling, and facilitating comprehensive care plans to ensure successful community re-entry for our residents. Your exceptional communication and organizational skills will enable you to work effectively with interdisciplinary teams, residents, families, and designated representatives.

Responsibilities:

• Assess psychosocial needs, adjustment to illness, and community re-entry barriers by completing comprehensive admission intake/psychosocial assessments, trauma-informed assessments, and comprehensive care plans per policy
• Provide individual, group, and family counseling
• Facilitate comprehensive care plans with interdisciplinary teams, resident neighbors, families, and designated representatives
• Explore discharge planning/community options, including TBI waiver, NHTD waiver, OPWDD waivers, and traditional discharge services
• Maintain open lines of communication with designated representatives and legal guardians
• Interface with in-house OASIS program, connections with the justice system, and maintain open lines of communication with all disciplines
• Complete entitlements/benefits and participate in Performance Improvement activities

Requirements:

• Bachelor of Social Work (BSW) degree or Bachelor degree in a related field with one year experience in social work or case management
• Professional attire and clinical communication skills
• Experience with office technologies, computers, word processing, and software programs
• Good organizational skills, pleasant and professional telephone persona, and ability to communicate well with department staff
• Must be able to read, write, speak, and understand the English language
• Ability to make independent decisions, possess patience, tact, cheerful disposition, and enthusiasm

Compensation:

• $25 - $30 hourly rate commensurate with experience
