Job summary

Northeast Center for Rehabilitation and Brain Injury is seeking a Discharge Planner to assess psychosocial needs and facilitate care plans to support residents’ community re-entry. You will counsel individuals and families, coordinate with interdisciplinary teams, and explore waivers and discharge options.

The role requires strong communication, organizational skills, and the ability to work with residents, families, and guardians in a supportive setting.

Qualifications

• Bachelor of Social Work (BSW) or related bachelor's with 1 year social work/case management experience
• Professional attire and clinical communication skills
• Experience with office technologies, computers, word processing, and software programs
• Good organizational skills and professional telephone persona
• Must read/write/speak English
• Independent decision making and enthusiastic demeanor

Responsibilities

• Assess psychosocial needs, adjustment to illness, and community re-entry barriers via intake/psychosocial assessments and comprehensive care plans
• Provide individual, group, and family counseling
• Facilitate comprehensive care plans with interdisciplinary teams, residents, families, and designated representatives
• Explore discharge options including TBI/NHTD/OPWDD waivers and traditional discharge services
• Maintain communication with designated representatives and guardians
• Interface with OASIS program and connections with the justice system and other disciplines
• Complete entitlements/benefits and participate in Performance Improvement activities

We are seeking a highly motivated and experienced Discharge Planner to join our team at Northeast Center for Rehabilitation and Brain Injury. Our 39-acre campus in Mid-Hudson Valley offers a unique and supportive environment for individuals with cognitive, physical, and social barriers.

Job Summary

As a Discharge Planner, you will be responsible for assessing psychosocial needs, providing counseling, and facilitating comprehensive care plans to ensure successful community re-entry for our residents. Your exceptional communication and organizational skills will enable you to work effectively with interdisciplinary teams, residents, families, and designated representatives.

Responsibilities

• Assess psychosocial needs, adjustment to illness, and community re-entry barriers by completing comprehensive admission intake/psychosocial assessments, trauma-informed assessments, and comprehensive care plans per policy
• Provide individual, group, and family counseling
• Facilitate comprehensive care plans with interdisciplinary teams, resident neighbors, families, and designated representatives
• Explore discharge planning/community options, including TBI waiver, NHTD waiver, OPWDD waivers, and traditional discharge services
• Maintain open lines of communication with designated representatives and legal guardians
• Interface with in-house OASIS program, connections with the justice system, and maintain open lines of communication with all disciplines
• Complete entitlements/benefits and participate in Performance Improvement activities

Requirements

• Bachelor of Social Work (BSW) degree or Bachelor degree in a related field with one year experience in social work or case management
• Professional attire and clinical communication skills
• Experience with office technologies, computers, word processing, and software programs
• Good organizational skills, pleasant and professional telephone persona, and ability to communicate well with department staff
• Must be able to read, write, speak, and understand the English language
• Ability to make independent decisions, possess patience, tact, cheerful disposition, and enthusiasm

Compensation

• $25 - $30 hourly rate commensurate with experience
