Hickory Trail Hospital is seeking a Discharge Planner to coordinate discharge planning across inpatient and outpatient services. The role emphasizes developing care plans to meet post-hospitalization needs and ensuring continuity of care without supervisory duties.

The position also serves as Court Liaison, coordinating documentation with Mental Illness Courts regarding OPCs, MIWs, APOWWs, and court-ordered medications, while adhering to CPI training requirements.

Qualifications

Position Summary:

The Discharge Planner is responsible for planning and coordinating activities related to discharge planning. Developing and implementing a plan of care designed to meet the patient’s post hospitalization needs while ensuring continuity of care. No supervisory function. This position also serves in the role of Court Liaison which will coordinate and complete all documentation with the Mental Illness Courts regarding OPC’s, MIW’s, APOWW’s, and court ordered medications.

Qualifications
Education:

Master’s Degree from an accredited college or university, in Social Work or a clinical related mental health field preferred.

Experience:

Minimum of three (3) years’ experience in a psychiatric health care facility with direct experience in discharge planning.

Additional Requirements:

Crisis Prevention Trainings (CPI) within 90 days of employment and prior to assisting with a restraining procedure.
