Responsibilities

• Acts as liaison between patient/hospital and outside agencies regarding discharge arrangements.
• Primarily but not limited to arranging post hospital encounter MD visits.
• May participate in departmental studies and projects to include planning and coordinating activities.
• Complies with hospital and departmental policies and procedures, including protecting the confidentiality of patient information.
• Performs other duties as assigned.
