Job summary

Jobtailor is seeking a qualified Social Worker to provide discharge planning, crisis intervention, and care coordination for patients and families in a hospital setting. The role includes comprehensive assessments, collaboration with care teams, and liaison to community resources.

The ideal candidate holds a Bachelor’s in Social Work, a state license, and at least two years of social work experience. Certification in Case Management is preferred.

Qualifications

• Bachelor’s Degree in Social Work or related field.
• Must possess current license as required by state board where services will be provided.
• Certified in Case Management (Preferred).
• Two (2) years' social work experience.

Responsibilities

• Provide social work services to patients and families through counseling, support, and group leadership.
• Complete comprehensive assessments and discharge planning with psychosocial considerations.
• Assess changes in physician plan and impact on discharge plan.
• Review and document discharge plans in the EMR and communicate changes.
• Identify problems with post-discharge care and initiate referrals to appropriate resources.
• Collaborate with hospital and community resources to meet patient needs and facilitate transfers.
