Job summary

PIH Health Hospital is seeking a discharge coordination specialist to provide advanced clerical support for the Care Management team. The role emphasizes forward-facing patient and family engagement, regulatory discharge documentation, post-acute coordination, and real-time tracking across electronic systems.

It supports timely, compliant discharge planning and acts as an operational extension of the RN Case Manager under delegated authority.

Qualifications

• High School diploma or equivalent.
• LA City Fire Card (PHGSH only).
• 1 year of administrative office, healthcare coordination, or customer service experience.
• Healthcare certificate/program or Associate degree, or higher will be considered in lieu of experience.
• Experience in Care Management, discharge planning, utilization review, or patient access preferred.
• Familiarity with Medicare discharge rights and post-acute care workflows preferred.

Responsibilities

• Provide advanced discharge coordination and clerical support for the Care Management team.
• Emphasize forward-facing patient and family engagement and regulatory discharge documentation.
• Coordinate post-acute care and real-time electronic tracking for timely discharge planning.
