Job summary

You will coordinate discharge teaching and follow-up, ensure high-quality patient experiences, and support ongoing clinical education across shifts. California RN license and BLS are required, with at least two years of acute care hospital experience.

Qualifications

• Two years recent clinical experience in an acute care hospital.
• Prior case management experience, preferred.
