Job summary

Mount Sinai Morningside seeks a Social Worker for Women & Children in ambulatory care. The role provides complex care management and coordinates services for underserved families under a grant.

You will collaborate with physicians, nurses, and social work colleagues, perform mental health evaluations, counseling, and data maintenance, with emphasis on family-centered care and adherence to primary care.

Qualifications

• NYS LMSW/LCSW license required.
• Experience with pediatric ambulatory care.
• Strong teamwork and organizational skills.
• Ability to collaborate with a large group of medical providers.
• Bilingual English/Spanish preferred.
• Minimum of one year in current position unless a Float in Women’s and Children’s.

Responsibilities

• Provide complex care management and supportive counseling.
• Perform primary mental health evaluations and short-term treatment.
• Collaborate with community providers and child protection when needed.
• Engage families during pediatric appointments and maintain data on cases served.
• Work with multidisciplinary teams of physicians, nurses, and staff.
