Job summary

MUSC Health seeks a Social Worker II (MSW) to support discharge planning and care transitions under the supervision of the Manager of Case Management. The role focuses on coordinating the discharge process to ensure a successful transition in collaboration with the health care team and achieving targeted patient outcomes.

Qualifications

• Bachelor's Degree or equivalent.
• 4-6 years of work experience.
• State licensure and/or professional certification dependent on position.

Responsibilities

• Assist with coordination of the discharge process to ensure a successful transition in collaboration with the health care team.
• Complete an assessment on the patient and caregivers during hospitalization and outpatient follow-up as needed.
• Identify risk factors that the patient may experience after discharge and apply discharge interventions to lower the patient risk.
• Bridge the discharge to outpatient provider with the patient and caregiver to address issues and questions.
