Qualifications

Responsibilities

• Prepare discharge packets for patients moving to skilled nursing facilities.
• Deliver durable medical equipment (DME) to patient rooms at discharge.
• Coordinate start of home care services and arrange transportation for discharge.
• Communicate with patients, families, payers and staff via email, phone, or secure messaging.
• Document in the medical record and support department projects and initiatives.
