Job summary

NurseDeck Inc. seeks a Patient Care Coordinator to oversee care management for acute inpatient populations.

You will collaborate with physicians, patients, families, nursing staff, utilization review teams, and other healthcare professionals to promote continuous care progression and cost-effective outcomes. The role includes coordinating with community providers and ambulatory case managers to support appropriate care transitions, safe discharges, and readmission prevention.

Responsibilities

• Coordinate discharge planning to ensure appropriate discharge plans for patients.
• Collaborate with physicians and the ancillary team to ensure seamless care.
• Engage with patients and families to develop and communicate discharge plans.
• Coordinate with rounding physicians to align on care plans.
• Ensure continuity of patient care progression for clinical and cost-effective outcomes.
• Coordinate with community providers and ambulatory case managers.
• Assist with selecting appropriate level and transition of care for safe discharge.
• Implement measures to prevent readmissions.
