Job summary

AbsoluteCare is seeking a Transitional Care Manager to join the integrated community care team. You will coordinate discharge planning, post-discharge care, and collaborations with inpatient facilities, health plans, PCPs, specialists, and community resources to ensure smooth transitions and avoid readmissions.

The role requires RN licensure, 3+ years in care coordination, travel up to 80%, strong communication, and proficiency with EMR and Office software; salary range is $85,000 to $90,000 per

Qualifications

• Must be a licensed RN in the state of practice.
• 3+ years of experience serving complex populations in community settings.
• Experience with discharge planning, care transitions, and case management in the community.

Responsibilities

• Meet with members during inpatient admission and develop PCCP.
• Call members post-discharge to review instructions and coordinate follow-up.
• Coordinate post-discharge plans with PCPs, specialists, home health and community resources.
• Communicate with AbsoluteCare team and providers to review discharge plans and barriers.
• Maintain PCCP and member contact per agency requirements and standards.
• Provide teach-back education on conditions, symptoms and treatment options.
