Job summary

Sheppard Pratt seeks a discharge planning specialist to work with an interdisciplinary team to coordinate aftercare for mentally ill patients. You will develop and implement discharge plans, coordinate with community resources, and support families through the transition to community care.

Ideal candidates have a related bachelor's degree or equivalent experience, with knowledge of psychiatric terminology and discharge planning. A robust benefits package accompanies this role.

Qualifications

• Bachelor's degree in a related mental health care field.
• Six to twelve months of relevant field or work experience in a psychiatric setting.
• Knowledge of psychiatric terminology, discharge and aftercare planning methods, and community resources.

Responsibilities

• Collaborate with the interdisciplinary treatment team to provide discharge planning.
• Maintain knowledge of agencies and resources for discharge planning and placement.
• Communicate with patients considering developmental needs.
• Develop and implement discharge and aftercare plans; facilitate community transition.
