CODAC Health, Recovery & Wellness, Inc. is seeking a Discharge Planner to coordinate hospital stays and discharge planning for members in collaboration with adult recovery teams and Utilization Management.

The role emphasizes proactive discharge assessments, comprehensive intake processing, and coordination with hospital staff and medical records. The position requires extensive knowledge of behavioral health, excellent communication, and strong documentation skills.

Qualifications

Masters or Bachelors degree in healthcare field required or with related experience as specified.
At least 1 year related experience for non-healthcare degrees and for certain healthcare tracks.
Discharge planning and intake documentation experience preferred.
Literate with clinical care coordination and documentation practices.
Arizona licensure or eligibility is preferred for nursing or PA roles.
Must have strong communication and documentation skills.

Responsibilities

Initiates discharge plans for new admissions and manages related documentation.
Coordinate intake paperwork including CORE Assessment, ISP, and release forms.
Collaborate with adult recovery team and hospital staff on discharge planning.
Provide post-discharge resources, referrals, and follow-up scheduling.
Liaise with medical records to obtain necessary information.
Develop outpatient aftercare plans and transitions of care.
Arrange transfers to transitional sites and ensure 7-day follow-up when possible.
On-call for hospital enrollments within 24 hours of notification.
