Job summary

NaphCare is hiring a Full Time Release Planner at the Pima County Detention Center in Tucson, AZ. The role involves coordinating discharge plans, medical follow-up, and community resource access to help inmates transition back into the community.

You will work with correctional staff and healthcare professionals to ensure seamless, patient-centered care while maintaining thorough records and compliance with policies. NaphCare supports growth and advancement within the company.

Qualifications

• Bachelor’s Degree in a human service field or related area.
• Must possess an active BLS CPR Certification (AHA)
• Knowledge in discharge planning and coordination with community service providers
• Excellent communication skills with patients, staff, and physicians
• Able to work independently and in a team
• Ability to interact with patients from diverse backgrounds including those with mental health or substance abuse issues
• Maintain confidentiality and abide by facility policies and legal requirements
• Criminal background check and drug screening required

Responsibilities

• Evaluate medical, mental health, and social needs of inmates to identify post-release needs
• Create individualized discharge plans addressing medical follow-up, medications, and community resources
• Collaborate with healthcare professionals, social workers, and community organizations for seamless transitions
• Maintain accurate records of patient interactions and follow-up activities
• Provide information and support to inmates and families regarding discharge planning and care options
• Act as liaison between the facility and community agencies to facilitate referrals
