Job summary

Qualifications

• Registered nurse with current NMC registration.
• Diploma plus specialist qualifications or experience in specialist area; prepared to pursue degree-level study.
• Strong IT and MS Office skills; experience with data platforms.

Responsibilities

• Be the case manager for complex discharge plans and provide specialist discharge advice.
• Coordinate with MDT, patients, families and external partners to ensure safe discharge.
• Utilise OPTICA data platform to track patient journey and resolve discharge barriers.
• Communicate effectively face-to-face, by phone and email to support timely discharge.
