Job summary

Community Health Network in Indianapolis is seeking a Case Management Extender to provide non-licensed clerical and administrative support to the RN and social work Case Managers, helping discharge plans and related activities.

Duties include calling or faxing information for patient care transitions, locating facilities for placement, arranging transport, and scheduling post-discharge physician appointments. Strong organizational and communication skills are essential.

Qualifications

• Three years of experience with medical/business computer applications.
• Proficiency in word processing and spreadsheet creation.
• Knowledge of medical terminology or completion of department approved training within three months of hire.

Responsibilities

• Provide clerical and administrative support to discharge plans and related case management activities.
• Call or fax information for patient care transitions (SNF, Home Health, assisted living, etc.).
• Assist with locating facilities for patient placement moving to another level of care.
• Facilitate transfers to alternate care facilities and arrange transport.
• Schedule post-discharge physician appointments.
