Job summary

The State of Florida, Sunland Division, is seeking a Case Manager for the Sunland program in Marianna, FL. The role involves coordinating interdisciplinary care, ensuring compliance with state standards, and supporting residents through assessments, planning, and discharge processes.

You will prepare psychosocial assessments, monitor budgets, assist with guardianship issues, and maintain ongoing communication with families and courts.

Qualifications

• Bachelor's degree from an accredited college/university with major coursework in social work or related field.
• Experience with persons with intellectual or developmental disabilities preferred; direct service experience valued.

Responsibilities

• Prepare initial psychosocial assessments for new admissions.
• Annually update psychosocial reports for caseload.
• Coordinate and interpretative communications with guardians and courts.
• Monitor residents' finances and assist with benefits applications.
• Advise and advocate for residents and guardians; assist in guardianship and court proceedings.
• Visit residents monthly and document changes and transitions.
• Coordinate referrals and placements; complete discharge processes and aftercare follow-ups.
• Support treatment goals through activities and data entry in ABC system.
• Assist in accompanying residents to medical appointments and support staffing needs.
