Job summary

CIDNY is hiring a Benefits Counselor for the NY Connects program in New York. The role provides information, intake, eligibility screening, and ongoing follow-up to help individuals access long-term services and supports, including developing person-centered independence plans.

The position is hybrid, based in CIDNY’s Bronx office, with three days in the office weekly, serving the Bronx and surrounding areas. Strong communication and computer skills are essential.

Qualifications

• Bachelor’s degree with 2+ years of benefits counseling experience.
• Associate’s degree with 5+ years of benefits counseling experience.
• Experience providing benefits counseling and related services.

Responsibilities

• Provide information, assistance, and referrals to consumers regarding benefits, services, and community resources.
• Conduct intake interviews and gather information necessary to assess consumer needs.
• Complete comprehensive benefits eligibility review, including explaining eligibility requirements and helping with applications/renewals.
• Provide Person-Centered Options Counseling for LTSS choices.
• Assist with benefits applications, recertifications, and referrals to programs.
• Collaborate with consumers to develop goals and monitor progress.
• Facilitate transitions from institutional settings to the community.
• Maintain complete consumer files and ensure HIPAA compliance.
• Contribute to CIDNY’s Resource Directory with updated information.
