Job summary

Palmer Home for Children seeks a Director of Transitional Care to oversee the existing program and design future models that promote independence for young adults. The role focuses on trauma-informed care, structured outcomes, and cross-functional collaboration with operations, compliance, HR, and development.

Qualifications

• Bachelor’s degree in social services, nonprofit leadership, education, program administration, or a related field is required; master’s degree preferred.
• Five or more years of progressive leadership, program management, or program development experience.
• Experience developing, implementing, or expanding programs from concept through execution.
• Experience serving young adults, transitional-age youth, or individuals moving toward independent living is strongly preferred.

Responsibilities

• Provide overall leadership and accountability for the day-to-day operation, quality, and continued development of Palmer Home’s Transitional Care program.
• Recruit, develop, and supervise Transitional Care staff, establishing clear expectations and fostering ownership, communication, collaboration, learning, and accountability.
• Ensure staff are trained to provide trauma-informed, relationship-based support promoting stability and progress toward independence.
• Establish and maintain program policies, procedures, documentation, service standards, and reporting.
• Oversee program budgets, expenditures, staffing, resources, and planning.
• Ensure compliance with laws, regulations, accreditation standards, Palmer Home policies, and best practices.
