HCA Houston Healthcare Kingwood is seeking a Director of Patient Safety to lead the hospital's safety program, drive zero-harm strategies, and partner with clinical leaders to build a culture of safety. The role requires CPPS, a healthcare background, and 3–5 years in patient safety; Master’s degree is preferred.

You will report to the Hospital Administration and collaborate across departments to sustain high reliability.

Qualifications

  • Bachelor’s Degree in healthcare related field required.
  • Master’s Degree preferred.
  • Clinical background preferred.
  • Three to five years healthcare experience in patient safety, risk, and/or quality (recent clinical setting preferred).
  • Certification in Patient Safety (CPPS) required; if not CPPS, certification must be obtained within 12 months of hire.
  • Maintain CPPS while in role.

Responsibilities

  • Lead and collaborate in implementing the hospital-based patient safety program with the goal of eliminating preventable patient harm and fostering a culture of safety.
  • Prevent future harm by initiating proactive evaluation and redesign of systems to improve care processes.
  • Support improved outcomes by emphasizing behaviors and robust systems with clear accountability.
  • Improve delivery of evidence-based care and reduce preventable harm through reliable design.
  • Reduce variation in care delivery; partner with PSO to explore identified variations.
  • Utilize alerts and best practices to perform gap assessments and implement actions.
  • Report, investigate, and analyze patient safety incidents, medical errors, and risks.
  • Facilitate serious event analysis for credible improvement strategies.
  • Conduct failure mode effect analysis to mitigate adverse outcomes.
  • Perform Patient Safety Rounds and empower staff to resolve safety concerns.
  • Coordinate disclosure of serious events to patients/families per policy and regs.
  • Ensure timely reporting of PSWP to the PSO and participate in learning collaboratives.
  • Champion Culture of Safety surveys and analyze results to drive action plans.
  • Support harm reporting in a nonpunitive environment and provide constructive feedback.
  • Oversee reviews addressing system and individual accountability.
  • Serve as chair or co-chair of the Patient Safety Committee.
  • Develop/input organizational policies related to patient safety and risk management.
  • Include patient safety in new hire orientation and ongoing education for leaders, medical staff, and staff.
