Qualifications

• Clinical licensure required (PT, OT, SLP or RN preferred).
• Two years of recent experience in acute care setting preferred.
• Must maintain acceptable driving record and insurability.

Responsibilities

• Lead and direct the Case Management Department, including staffing, education, and mentorship.
• Manage patient outcomes data, PPS documentation, and FOM credentialing.
• Attend Operations/Admissions meetings and coordinate with HIM on coding and compliance.
• Support clinical liaison and physicians to ensure accurate documentation and CMS compliance.
