Job summary

Sequim Bay Post Acute in Sequim, WA is seeking a Director of Social Work to lead our social services department and advocate for residents and families throughout admission to discharge with compassion.

The role emphasizes leadership, care planning, discharge coordination, benefits guidance, and collaboration with an interdisciplinary team. A Bachelor's in Social Work is required, MSW preferred; 1+ year of health care social work experience is expected.

Qualifications

• Bachelor's degree in social work or related field is required.
• MSW preferred for advanced practice and leadership.
• 1+ year of supervised social work experience in a health care setting.
• Current social services certification or licensure as required.
• Experience documenting in an EMR and strong computer skills.
• Excellent communication and ability to collaborate with residents, families and staff.

Responsibilities

• Lead the social services department, setting priorities and ensuring residents' psychosocial needs are met.
• Support admission by meeting with prospective residents, families, and responsible parties.
• Complete assessments and planning in EMR within seven days of admission and maintain progress notes.
• Develop care plans with measurable goals for social service needs.
• Guide discharge planning, including placement, home health, and transportation.
• Assist residents with benefits and financial matters such as Medicare, Medicaid, and VA benefits.
• Keep families informed about health status, goals, and discharge plans.
• Support residents in choosing their health care decisions and advance directives.
• Provide or arrange counseling and support for grieving residents.
• Coordinate referrals to outside agencies and obtain adaptive equipment and personal items.
• Oversee staff and coach on meeting individual resident needs.
