Job summary

Sutton Park Center for Nursing and Rehab is seeking a Social Worker to conduct MDS assessments and lead care plan meetings and discharge planning. The role involves implementing advanced directives and ensuring accurate documentation in the EMR, with a strong emphasis on safety and confidentiality.

Ideal candidates have an MSW or BA/BS with licensure, and prior long-term care experience is preferred. This is a full-time, in-person position with competitive benefits.

Qualifications

• Bachelors or Master of Social Work preferred.
• Social Work License required.
• Prior experience in a Skilled Nursing Facility or Long-Term Care setting preferred.

Responsibilities

• Complete MDS assessments.
• Facilitate care plan meetings and discharge planning.
• Implement History and Assessment, Advanced Directives and EOL planning.
• Audit psychology paperwork, hospital discharges, room changes, PASRR and Advanced Directives.
• Complete all required documentation in the EMR.
• Coordinate care and services with patient-centered plans and strong clinical/communication skills.
• Demonstrate good judgment of safety and confidentiality issues.
• Maintain strong verbal and written communication.
• Provide strong customer service.
• Know community resources.
• Have working knowledge of Sigmacare.

• Coordination of care and services including ongoing monitoring of an appropriate and effective person-centered care plan; Strong Clinical, Communication and Organizational Skills

• Knowledge of the community resources
• Working Knowledge of Sigmacare

* Good working knowledge of long term care regulations including documentation requirements
