Job summary

EHG seeks a Director of Social Work to lead our social services team in a Virginia elder care facility. You will oversee the department, conduct psychosocial assessments, and craft individualized care plans to support residents and families.

• Bachelor's degree required; master's degree preferred.
• Current Social Work license or certification as required by state regulations.
• Experience in long-term care, skilled nursing, rehabilitation, or healthcare preferred.

Responsibilities

• Direct and oversee the facility's Social Services Department.
• Complete psychosocial assessments and develop individualized care plans.
• Assist residents and families with adjustment, crisis intervention, and long-term planning.
• Coordinate discharge planning and transitions of care.
• Facilitate resident and family meetings as needed.
• Ensure compliance with federal, state, and local regulations.
• Collaborate with interdisciplinary teams to promote resident well-being and quality of life.
• Assist residents in accessing community resources, benefits, and support services.
• Maintain accurate documentation and participate in care plan conferences.
