Job summary

EHG Social Services Director role overseeing residents' social, emotional, and psychological needs, coordinating discharge planning and care planning across the facility.

The position requires a bachelor’s degree in social work or related field, at least one year of supervised health-care social work experience, and strong English communication. You will lead a team, ensure regulatory compliance, and partner with the mental health provider to support residents and families.

Qualifications

• Bachelor's degree in social work, sociology or psychology as required.
• One year of supervised social work experience in health care.
• Approval from Home Office if no degree or if degree not listed.
• Approved social work experience in health care.
• Strong verbal and written English communication.

Responsibilities

• Meet physical and sensory requirements and work in the described environment.
• Identify and participate in process improvements to enhance customer experience and workflow.
• Lead management duties including hiring, training, developing, coaching, and counseling staff.
• Ensure corrective action is fair and compliant with laws and policies.
• Complete Social Services portions of MDS and related care plans and notes.
• Assist residents and families with social, emotional, and family issues.
• Participate in resident care planning per medical assessment.
• Maintain progress notes and discharge planning per policy.
• Make referrals to support agencies and document them.
• Coordinate Family Council Meetings and resident room changes.
• Collaborate with mental health provider to address psychosocial needs.
• Educate community about aging, rights, and facility services.
• Orient residents to long-term care and facilitate adjustment.
