Delaware Oaks in Buffalo, NY seeks a compassionate care coordinator to interview participants and families, perform psychosocial assessments, and tailor care plans to individual needs.

The role includes helping clients access social services, providing crisis and grief counseling, and improving communication among participants, families, and staff. A Bachelor’s degree is required, long-term care experience is essential, and NYS Screen Certification is preferred.

Formación

• A current/valid Bachelor's Degree.
• Long Term Care exp. is a MUST.
• NYS Screen Certification preferred.
• Strong admission and discharge planning experience.

Responsabilidades

• Interviewing participants and their families.
• Provide psychosocial assessment and counseling to participant and families.
• Developing specific care plans based on the needs of individual patients.
