Job summary

Caretree Health Corporate in Buffalo, NY is seeking a dedicated Social Worker to support participants and families through psychosocial assessment, counseling, and the development of individualized care plans.

The role emphasizes coordinating with staff, facilitating access to social services, crisis intervention, grief counseling, and smooth admission and discharge planning. A bachelor's degree is required, LTC experience is essential, and NYS Screen certification is preferred.

Qualifications

• A current/valid Bachelor’s Degree.
• Long Term Care experience is required.
• NYS Screen Certification preferred.
• Strong admission and discharge planning experience.

Responsibilities

• Interviewing participants and their families.
• Provide psychosocial assessment and counseling to participant and families.
• Developing specific care plans based on the needs of individual patients.
• Helping participant apply for appropriate social services.
