Job summary

Transforming Age’s Mercy Retirement & Care Center in Oakland is seeking a Social Services Director to lead resident support, assessments, and care planning across Health Center and Assisted Living. The role focuses on identifying social, psychological, and emotional needs and coordinating services to meet them.

The position requires 3–5 years of social work experience, a BA in Behavioral Science (or a related field) and preferably a Master in Social Work or related discipline.

Qualifications

• Masters preferred; BA/BS in Behavioral Science acceptable.
• 3–5 years of experience as a social worker.
• Must be 21 years or older with five years in a social services role.
• Experience in geriatrics, Medicaid, Medicare, and third-party reimbursement.
• Excellent communication, leadership and organizational skills.

Responsibilities

• Identify social, psychological and emotional needs of residents.
• Arrange services to meet residents' needs and support adjustment.
• Interact with families and residents in Health Center and Assisted Living.
• Document care plans, assessments, and meetings.
• Coordinate transfers and participate in transitional care meetings.
• Maintain policies and procedures for the Social Services department.
• Promote Transforming Age culture of excellence in all areas of responsibility.
