Job summary

THRYVE Healthcare is seeking a dedicated Social Services Director to lead our Social Services Department and support residents’ social, emotional, and psychological well-being. You will work with residents, families, leadership, and the interdisciplinary team to ensure residents receive resources and advocacy throughout their stay.

The role requires a Bachelor’s in Social Work and at least 2 years of social work/case management experience in a skilled nursing facility, with supervisory skills

Qualifications

• Bachelor’s degree in Social Work or related field required.
• Minimum 2 years of experience in social work or case management within a skilled nursing facility required.
• Supervisory or leadership experience preferred.
• Strong knowledge of social work principles, long-term care, discharge planning, and community resources.
• Excellent communication, interpersonal, organizational, and problem-solving skills.
• Ability to work effectively with residents, families, staff, and interdisciplinary team members.
• Compassionate, professional, and resident-focused approach.
• Ability to maintain confidentiality and handle sensitive information.

Responsibilities

• Develop, implement, and oversee social services programs to meet residents and families' needs.
• Complete psychosocial assessments and identify residents’ social, emotional, and psychological needs.
• Collaborate with interdisciplinary teams to create individualized, resident-centered care plans.
• Provide counseling, support, advocacy, and interventions to residents and families.
• Lead and supervise social services staff with guidance and training.
• Assist residents and families with adjustment to the facility and changes in health or independence.
• Coordinate referrals to community resources and outside agencies as appropriate.
• Support discharge planning and safe transitions of care.
• Develop relationships with community organizations and service providers.
• Ensure timely documentation of activities and resident progress.
• Monitor department performance and identify improvement opportunities.
• Ensure compliance with applicable regulations and facility policies.
• Participate in care plan meetings and QA activities.
• Maintain confidentiality and protect residents’ rights.
