Qualifications

• Experience with MDS 3.0 in a long-term care or sub-acute setting.
• Proficient in discharge planning and memory support.
• Experience in long term care/sub-acute environments.
• Familiarity with EMR and basic computer programs.

Responsibilities

• Manage and oversee the entire Social Service Department.
• Identify individual social and emotional needs of residents.
• Develop and maintain individualized care plans.
• Participate in admissions and discharge planning to the community.
• Participate in interdisciplinary care plan meetings to address social needs.
• Assess residents regularly.
• Coordinate investigations for resident grievances.
