Job summary

Heritage Manor Care in Anaheim, CA is seeking a full-time Director of Social Services to provide comprehensive social work services to residents, oversee psychosocial care, and coordinate admissions and discharges. You will collaborate with nursing and other departments to support residents’ quality of life and discharge planning.

The role requires a Bachelor's degree in Social Work or related field, 1+ year in skilled nursing, and SSD certification; a Master’s degree is a plus.

Qualifications

• Bachelor's degree in Social Work, Human Services, Sociology, Education, Psychology, Rehabilitation/Counseling or related field required.
• Minimum of one year of experience in skilled nursing settings and management experience.
• SSD Certification is required.

Responsibilities

• Coordinate admissions and discharge procedures for residents.
• Attend Interdisciplinary Team and Unit Care conferences; assess residents for psychosocial groups; supervise implementation and documentation.
• Interview new residents and assess psycho-social condition; participate in care conferences providing social information.
• Oversee psychoactive committee review with the Director of Nursing.
• Identify and address resident problems; implement solutions; monitor follow-up and discharge planning.
• Deliver in-service programs related to Social Services functions when requested.
• Make counseling referrals to outside agencies as needed and maintain cross-department communication.
