Qualifications

• Bachelor's degree in Social Work or related field.
• Minimum 1 year of experience in a nursing facility.
• NJ license or certification in Social Work.

Responsibilities

• Conduct social work assessments to address residents' physical, emotional, and psychological needs.
• Develop and implement care plans to address residents' identified needs.
• Collaborate with the interdisciplinary team to provide comprehensive care and services.
• Provide counseling and support to residents and families to promote emotional well-being.
• Maintain accurate and timely records of resident assessments, care plans, and progress notes.
• Participate in care conferences and other meetings to discuss resident care and services.
