Oak View Health and Rehabilitation in the United States is seeking a dedicated social worker to lead social services within a skilled nursing facility, coordinating care plans and supporting residents and families.

The role emphasizes community relations, discharge planning, and collaboration with staff to ensure compassionate, compliant care. A bachelor's degree in social work or a related field is required, with experience in SNF settings preferred.

Qualifications

• Bachelor's degree in social work or a human services field (including sociology, special education, rehabilitation counseling, psychology).
• Social work experience in a skilled nursing/long-term care setting working directly with individuals.
• Ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
• Leadership ability and willingness to supervise other personnel.

Responsibilities

• Develop, implement, evaluate and direct the social service programs of the facility.
• Participate in community planning related to the interests of the facility and the services and needs of the resident and family.
• Maintain community relationships to help address resident concerns as appropriate.
• Perform routine tasks with patient admissions, care plans and discharge planning.
• Coordinate response to reports of missing, lost or stolen belongings, along with other grievances.

• Must possess leadership ability and willingness to work harmoniously with and supervise other personnel.
