Job summary

Augusta Center for Health & Rehabilitation in Augusta, ME, seeks a Director of Social Services to lead our Social Services Department and care programs for residents. You will guide a team to ensure emotional, social, and psychological well-being while coordinating with families and external resources.

The role requires LICSW/LSW licensure preferred, 4+ years in social services with 2+ in leadership within long-term care, and a strong understanding of aging and chronic illness to advocate for

Qualifications

• Licensed Social Worker (LSW) or Clinical Social Worker (LCSW) licensure preferred.
• Minimum of 4 years in social services, with at least 2 years in a leadership role in a long-term care setting.
• Strong understanding of aging, psychosocial needs, and chronic illness.

Responsibilities

• Develop and implement comprehensive social services programs addressing residents' diverse needs.
• Provide leadership to the Social Services team ensuring high-quality, person-centered care.
• Collaborate with interdisciplinary teams to create individualized care plans for residents.
• Liaise with residents, families, and community resources to support transitions and discharge planning.
• Advocate for residents' rights and maintain their dignity and independence.

As a Director of Social Services, you will lead our Social Services Department and the programs within it. Your work will play a crucial role in ensuring the emotional, social, and psychological well-being of our residents.

Key Responsibilities of a Director of Social Services:

• Develop and implement comprehensive social services programs that address the diverse needs of our residents
• Provide leadership to the Social Services team, ensuring the delivery of high-quality, person-centered care
• Collaborate with interdisciplinary teams to create individualized care plans that enhance the overall well-being of residents

Rate of Pay

USD $62,000.00 - USD $74,000.00 /Yr.

What You'll Bring:

Qualifications of a Director of Social Services:

• Licensed Social Worker (LSW) or Clinical Social Worker (LCSW) preferred
• Minimum of 4 years in social services, with at least 2 of those years in a leadership role in a long-term care setting
• Strong understanding of the aging process, psychosocial needs of the elderly, and the impact of chronic illness
• Ability to collaborate effectively with a diverse team and build positive relationships with residents and their families
